As endocrinologists, we deal with metabolic diseases, such as diabetes, thyroid disease, osteoporosis, abnormal cholesterol and many others. As specialists, we must ask that you have a primary care physician serve your more routine medical needs.
What to bring with you:
Many changes can take place in a patient’s care in between visits to their doctor. With this in mind we request that our patients bring the following items to every office visit:

Bring all current insurance cards and a photo ID to every visit.

Obtain any referrals required by your insurance carrier from your primary care physician prior to all visits.

Bring all medication bottles or an updated medication list to each office visit. This includes any over the counter supplements or vitamins.

Diabetics should bring their glucose monitors and any blood sugar logs.
New patient forms
Each new patient needs to fill out a New Patient Registration form, Consent for Treatment / Privacy Notice, Medical History form & Health Questionnaire.
Anyone being referred for the condition of diabetes will also need to fill out our Diabetic Questionnaire. These forms are provided here for convenience. They are PDF documents that are nine pages each. You can print them off and complete them at home before arriving at your appointment.
Appointment cancellations
If you must cancel an appointment, we’ll be happy to reschedule at a more convenient time, but we ask that appointments be cancelled at least 24 hours in advance, permitting us to better serve other scheduled patients
For caregivers
Please make sure we have copies of any Living Will or Durable Power of Attorney on file. It is also important to list those persons we can discuss information with on your New Patient Registration form
